The responsibility of paying for our services lies with you. However, if you have insurance coverage and have met your deductible, we will file your insurance after you provide us with the proper information and insurance form.

Montgomery Family Practice is a participating physician group for Medicare patients. This means we accept the assigned benefit for all services. Your out-of-pocket expense is limited to the annual deductible and the 20 percent co-payment mandated by Medicare. The patient co-payment may be covered by Blue Shield or another secondary insurance.

Montgomery Family Practice also participates in several HMO and PPO plans, except Anthem Senior Advantage, Medicaid & Choicecare Select. Please call the office to check on your specific plan. We realize that insurance policies are sometimes difficult to understand so we are available to answer your questions.

Co-payments on insurance plans or payment for services not covered by your health insurance plan are requested on the day of your visit.
